General medical care at Medina Health Ministry includes physical examinations and treatment covering a full range of basic medical problems. All diagnostic testing and treatment for qualified MHM patients, including blood work, x-rays, physical and occupational therapy, dietician services, diabetic rehabilitation and in-patient service are performed, free of cost, ONLY at Medina Hospital.
Prescriptions
Various methods of helping patients with their medications can be implemented. Our volunteers will assist you in applying for pharmaceutical patient assistance programs.
Medina Health Ministry Regulations
The following regulations apply to all Medina Health Ministry patients:
You must have an appointment to be seen.
Patients become ineligible for Medina Health Ministry services when they become qualified for any government medical care assistance or aid, insurance coverage, or income exceeding MHM guidelines.
A patient, who does not keep a clinic appointment without canceling with the MHM staff, may no longer be eligible for services.
Emergency Room visits are not part of the MHM services. In an emergency, patients are advised to go to the emergency room to seek immediate treatment. Patients should advise the Medina Hospital registration department of their MHM patient status. Please be aware that charges may incur.
MHM reserves the right to dismiss a patient at anytime.
Referrals
Please do not call the office about your referral.
A staff member of MHM will contact the referral physician by letter. Once the specialist contacts MHM, patients will be provided the telephone number for the referral physician that has agreed to provide continued care.
The patient is responsible for making an appointment with the referral physician.
The patient is responsible for any charges at Medina Hospital, which have not been authorized by a MHM physician and for any charges from non-MHM physician.
Referrals for continued care or further evaluation are made at the discretion of a MHM clinic physician or staff member. If the patient chooses not to see the recommended referral physician, MHM is not obligated to provide another referral physician or service.
If a patient does not keep a referral appointment and does not call and cancel, the patient is no longer eligible for MHM referral services for that specific medical problem and any services obtained become the financial responsibility of the patient.
